Risk Factors for Recurrence of <b>Depression</b> | Brain Blogger

Depression is a common mental health disorder that negatively affects daily functioning and quality of life. Unfortunately, depression is often a recurrent disorder. Current treatment guidelines are based on clinical features of the disease, but new evidence points to sociodemographic and psychosocial characteristics that influence the severity and prognosis of depression. Specifically, a study published in the Canadian Medical Association Journal highlights smoking and a feeling of lack of control over one’s life as significant predictors of a negative long-term prognosis of depression.

The population-based survey evaluated nearly 600 people aged 16 or older who participated in a national health survey. The national survey included more than 17,000 participants, and the subjects in this evaluation were those who had reported a major depressive episode in the first year of the survey. The participants were followed for six years. More than half (64.5%) of the participants were women, and, at baseline, the average age of the group was approximately 39 years. Most of the participants were white (91.7%) and earned a middle to high income (81.5%). Nearly two-thirds had achieved greater than a high school education.

Over the follow-up period, slightly more than half (55.3%) of the survey population experienced repeated episodes of depression. The authors evaluated numerous physical, emotional, psychological, social, and demographic characteristics of the participants to examine their association with depressive episodes. Daily smoking and low mastery (feeling that life circumstances are beyond one’s control) were significant predictors of recurrent depression. A history of depression, which is already a well-established risk factor for future episodes of depression, was also significantly related to recurrent depressive episodes in this study. The association between risk factors and depressive episodes varied with the severity of depression.

Due to the nature of the study and the small sample size, the direction of effect and causal mechanisms are unclear. However, interventions that promote smoking cessation and increase mastery may provide positive mental health outcomes. Smoking cessation has obvious health benefits, and high levels of mastery are linked to regular physical activity, a healthy diet, decreased metabolic risk factors, and a decreased risk of death from cardiovascular factors. In fact, high levels of mastery at baseline were protective against recurrent episodes of depression in this population.

Smoking and mastery are simple and straightforward to assess in routine clinical practice. According to the authors of the current study, these novel prognostic factors may offer new avenues for treatment of depression and prevention of recurrent depressive episodes.

Depression is the most common psychiatric disorder among HIV-positive peopleADDIS ABABA, 7 December 2011 (PlusNews) - HIV patients in Africa frequently suffer shame and depression but the continent’s health systems are ill-equipped to handle the issue, which not only affects their quality of life, but can lead to poor adherence to HIV treatment regimens.

While HIV programmes focus heavily on reducing externalized stigma and ill-treatment of HIV patients by society, little is done to deal with a patients’ self-perception and how that might deteriorate following an HIV diagnosis, speakers said at a session on stigma at the 16th International Conference on AIDS and Sexually transmitted infections in Africa in Addis Ababa.

Studies show that depression is the most common psychiatric disorder among people living with HIV, and is more prevalent among HIV-positive people than the general population.

"Operational research carried out in Zambia has found a positive correlation between patients who self-stigmatized and failure to adhere to treatment," said Sikazwe Izukanyi from Zambia’s Ministry of Health. "Self-stigma was often found in patients who did not disclose their status to partners or family members - making it difficult to maintain strict adherence to regimens while trying to hide the drugs."

Izukanyi noted that while counselling was a standard part of HIV care in Zambia, counsellors needed to be made aware of the prevalence of self-stigma and how to deal with it.

A 2010 Ugandan study by Makerere University found that HIV-positive patients were more critical of themselves, had significantly greater problems making decisions, poorer sleep, tired more easily, experienced more appetite changes and had more cognitive impairment.

ARVs and self-stigma

According to a study by Yordanos Tiruneh, an Ethiopian academic with US-based Northwestern University, antiretroviral (ARV) therapy has been key to reducing external stigma by minimizing the visibility of physical imperfections and restoring functional daily activities such as the ability to work. The study, which used 105 interviews with Ethiopian men and women on ARVs, also found that the support networks formed by people living with HIV gave them much-needed social capital.

However, according to Yordanos, while ARVs were linked to a reduction in external stigma, the study found that they tended to increase internalized stigma, sometimes resulting in failure to properly adhere to ARVs.

"When I think of the two tablets that keep me alive, I hate myself and I feel that I am dead," one of the study’s interviewees is quoted as saying. "Sometimes I get furious to see myself like a walking corpse, and other times I see myself as a doll that functions with a battery. I would say, without these batteries [pills], I am nothing."

According to a US study, adherence to ARVs was higher in patients for whom anti-depressants were prescribed.

A severe shortage of mental health professionals in Africa means that HIV-associated depression is largely ignored. For instance, according to the UN World Health Organization, Burundi has just one psychosocial care provider per 100,000, against a target of at least eight, while Ethiopia has less than one, against a similar target.

"The problem is largely a human resources one; while strengthening health systems, governments should remember to focus on mental-health issues," said Izukanyi. "As it is, we have no systems for screening, diagnosing and treating patients with mental-health issues."

Among other things, experts recommend integrating mental-health services into primary healthcare activities, developing mechanisms to ensure a good supply of psychotropic medication and more research into mental-health issues in Africa.

&#39;Love Hormone&#39; May Buffer Kids From Mom&#39;s <b>Depression</b> - Health <b>...</b>

FRIDAY, Dec. 9 (HealthDay News) — Children born to mothers with postpartum depression are at increased risk for mental health problems, but a hormone called oxytocin may reduce the risk, according to a new study.

Oxytocin, which is produced naturally in the body and has been associated with feelings of love and trust, may help protect kids from the negative effects of maternal depression, the researchers found. A synthetic version of the hormone is available as medication.

In the study, Israeli researchers looked at 155 mother-child pairs. By the time they were 6 years old, 60 percent of children born to mothers who were consistently depressed for the first year after giving birth had mental health problems, mainly anxiety and conduct disorders.

Among the 6-year-old children whose mothers did not have postpartum depression, only 15 percent had mental health problems, the investigators noted.

The study also found that children born to mothers with extended postpartum depression were less verbal and had lower levels of playfulness and creativity, less engagement with their mothers, diminished social involvement, and less empathy for the pain and distress of others.

These children and their mothers also had disordered functioning of the oxytocin system, as shown by lower levels of oxytocin in their saliva and a variant on the oxytocin receptor gene that increases the risk of depression, according to study leader Ruth Feldman, a professor in the psychology department and the Gonda Brain Sciences Center at Bar-Ilan University, and colleagues.

Among the children born to depressed mothers, the 40 percent who did not have mental disorders by age 6 had normal functioning of the oxytocin system and normal levels of oxytocin in their saliva.

The study was slated for presentation Thursday at the annual meeting of the American College of Neuropsychopharmacology, in Hawaii.

“We found the functioning of the oxytocin system helps to safeguard some children against the effects of chronic maternal depression,” Feldman said in a college news release. “This study could lead to potential treatment options for postpartum depression and methods to help children develop stronger oxytocin systems.”

Because this study was presented at a medical meeting, the data and conclusions should be viewed as preliminary until published in a peer-reviewed journal.

The Worldwide <b>Depression</b>/Recession Of 2012 | ZeroHedge

In case you haven't noticed, the rest of the world continues to slow down and the negative data is accelerating. The big powerhouses of the world, the eurozone including Germany, Japan, and China are leading this trend and there is no reason to believe that the U.S. will not follow.

I've been writing about this theme frequently lately because, while we are seeing some positive numbers here in the U.S., we are also seeing signs of weakness starting to show up, and since we live in a world of international trade, the world's woes will hit us.

The first thing to note about this phenomenon is that the central banks of the world, including the Fed, have been doing all they can to support their economies with plentiful money. According to a recent Bloomberg article, "Central banks across five continents are undertaking the broadest reduction in borrowing costs since 2009 to avert a global economic slump stemming from Europe’s sovereign-debt turmoil."

Monetary easing will push the average worldwide central bank interest rate, weighted for gross domestic product, to 1.79 percent by next June from 2.16 percent in September, the largest drop in two years, according to data and projections from JPMorgan, which tracks 31 central banks. The number of those banks loosening credit is the most since the third quarter of 2009, when 15 institutions cut rates, the data show. ...

The People’s Bank of China has raised its main interest rate three times this year to fight inflation. India’s central bank lifted rates on Oct. 25 by a quarter of a percentage point, while signaling it was nearing the end of its record cycle of increases as the economy cooled.

This is nothing new. Since the Crash of 2008, most central banks have been pumping fiat money into their economies.

The multiple EU sovereign insolvencies—you don't need to default to be insolvent—are hitting eurozone credit hard, which is a trigger for deflation as money supply declines. Lenders are stuck with bad sovereign loans and there isn't enough money to bail them all out, much less the PIIGS.

The thing to remember about the eurozone is that it's not just sovereign insolvencies that is their problem. They became insolvent, yes because governments spent too much, but also because their economies are in the tank mainly for many of the same reasons the U.S. economy declined (money inflation boom, high debt/spending, housing market collapse triggering depression, high taxes and regulation, and various bailouts to prevent recovery). Until they fix the underlying causes, their banks will collapse and countries will default.

China's economy relies on the West for its exports, and as a result:

Manufacturing production falls at fastest rate in 3 months; the lowest service sector growth since August;Overall input costs fall for the first month since July 2010;Service sector business optimism second-lowest in series history.

China's frequent monetary stimulus, along with government real estate policies, keeps feeding their real estate boom-bust cycles.

Once you look at the data, below, you will see where we and the world are headed. 

While these economies are shrinking, demand for commodities, capital goods, and manufactured goods all decline. This is impacting commodity prices; they have been falling since this summer (mainly a supply-demand factor, not just a money deflation issue). Each country/zone will have a different reaction to all this. Most will continue to inflate ("print" money).

Money printing will have little impact on declining prices for the time being. Unless they panic. If they panic, that is, massively pump money, they will suffer from price inflation. China will have more booms and the eurozone will also stagnate as well. Japan will continue to go "Japanese", and depending on what the Fed does, it is likely we'll go Japanese as well.

N.B. The word "depression" frightens a lot of people. It should. But, we are in one now. Our leaders just invented the word "recession" to take our minds off what's really happening. Murray Rothbard in his book, America's Great Depression noted that when the economy crashed again in 1937, FDR and his advisers didn't want to use the "D" word so they came up with "recession." Until that time there were no "recessions." Now a "recession" is just a mini-depression. Since we, in my opinion, have not yet recovered from the Crash of 2008, we are in a depression. Just ask the 25 million Americans who either don't have a job, can't find one, stopped looking, or are working part-time because they can't find full-time work. Just ask the 24% of home owners whose homes are financially underwater.

<b>Depression</b>, Disability Can Follow ICU Care: Study - Health News <b>...</b>

FRIDAY, Dec. 9 (HealthDay News) — Depression and new physical problems are common among patients released from the intensive care unit after treatment for a potentially deadly condition called acute lung injury, a new study finds.

The findings may also apply to ICU patients with other types of disease or injury who get hooked up to breathing machines, according to the researchers.

“When people are discharged from the ICU, we tend, understandably, to focus on their physical health, but our data tell us we need to focus on their mental health, too,” study leader Dr. O. Joseph Bienvenu, an associate professor of psychiatry and behavioral sciences at the Johns Hopkins University School of Medicine, said in a Hopkins news release.

“Depression can make recovery much more difficult. Identifying depressive symptoms early — and treating them — could make a real difference in how patients fare physically in the long term,” he added.

The researchers looked at the depression levels and physical abilities of 186 survivors of acute lung injury three, six, 12 and 24 months after they became ill. Physical abilities included whether people could perform basic tasks of daily living such as shopping, preparing food and using the telephone.

Forty percent of the patients developed depressive symptoms during the two years of follow-up, even though they had not previously experienced such symptoms.

In addition, 66 percent of the patients developed new physical impairments during the follow-up period.

The average age of the patients in the study was 49, and they should be in the prime of their lives, the researchers noted. Instead, many had become disabled and unable to return to work.

The researchers will continue to follow these patients to find out if the depression symptoms and physical impairments persist beyond the initial follow-up of two years.

The study was published Dec. 9 in the American Journal of Respiratory and Critical Care Medicine.

Guest post: Breastfeeding through postnatal <b>depression</b> | Circus <b>...</b>

Ella from Purple Mum is mother to three children and has had to deal with Postnatal Depression after each of their births. The illness has impacted her breastfeeding experience differently each time. She’s currently battling through and blogging about PND for the third time but generously took the time to share a thing or two about breastfeeding through it.

I have experienced postnatal depression following the birth of all three of my babies. During treatment for my illness it has been necessary for me to receive inpatient treatment. During this treatment I have met many mothers who have stopped breastfeeding either due to their illness or to medication choices.

Breastfeeding rates in the UK are, in my opinion, not ideal. Only 44.9% of mothers in the UK are still breastfeeding after six weeks. I’m not aware of any statistics regarding mothers with postnatal depression. Due to my own experiences and talking to other mothers on the ward where I was treated I can only assume that these figures are even lower than the general population.

Breastfeeding does not come naturally to every mother/baby pair. There is a learning curve. Until it is settled and comfortable breastfeeding can be at best bothersome. At worst – frustrating and painful, and could trigger postnatal depression in those already susceptible.

When breastfeeding is not going well pressure can come from all angles. If the baby is not gaining weight, well-meaning health professionals can cause a lot of stress. Family and friends wanting to help will make suggestions (such as switching to formula or continuing against the odds) which could be very upsetting for a new mother.

When a mother is determined to breastfeed but needing to supplement life can become one long cycle of feeding, expressing, and supplementing. There is little time left to adjust to becoming a new mum let alone enjoy the baby. This is what happened to me when I was trying to breastfeed my first baby. I really believe this played a large part in my developing postnatal depression.

If breastfeeding really does not work out it can have a terrible impact on a mother’s mood. Particularly if she felt very strongly about wanting to breastfeeding and the benefits.

From my own personal experience when at six weeks I switched my eldest child to formula I felt terribly guilty. I felt I had failed him, and failed at one of the fundamental tasks of becoming a mum.

However, I think if managed well, success achieved breastfeeding can have an incredibly positive effect on a mother’s mood. I know that for me, establishing and continuing breastfeeding my second and third children has helped me feel a sense of achievement. It’s gratifying to look at your thriving baby with the pride that you have nourished it.

When a mother breastfeeds oxytocin is released. This chemical causes feelings of relaxation and well-being which must be helpful for a mother suffering from postnatal depression.

Of course, the other major consideration for a breastfeeding mother suffering from postnatal depression is medication. Drug treatment is often recommended as it can speed up recovery and prevent relapse. On the ward I met mothers who had stopped breastfeeding in order to take the medication they were prescribed.

There are medications that can be prescribed for breastfeeding mums. However as research on infants is not possible (ethically) a lot of the information about the impact of these drugs is anecdotal. Also since some psychiatric medications are relatively new the data about them is short term so we cannot know the long-term affects on the baby.

This leaves the mother with a difficult choice in which she has to balance the severity of her illness with the potential risks to baby.

So as you can see whether to continue to breastfeed with postnatal depression is a very difficult question to answer. I think it is a very individual choice that should be made by the mother based on a variety of factors.

I do think that whatever choice is made it should be one that the mother can feel comfortable with. She shouldn’t feel guilty for needing to stop breastfeeding, there is more to an infants well being than the way they are fed.

Mum being well and happy has to be a number one consideration as this will have the biggest impact on her
baby.

If it is important to her to breastfeed and it is possible to continue I think she should be offered support and encouraged to do so. After all, this is the ideal situation.

Holiday Depression Is Basically GuaranteedThe Cleveland Clinic has issued a list of risk factors for holiday depression, and if you are alive, you basically have all of them.

According to USA Today, the risk factors include the following:

Connecting the holidays with family problems or painful memories.

Did a bad thing ever happen to you around the holidays? Or did a bad thing happen to you at some point, but you associate it with the holidays because that is when you are back in your hometown and forcibly confronted with all your memories? Go ahead and check the box next to this one.

Expecting that you should feel happy.

You mean like what happens when every store is playing, extremely loudly, a song that says "it's the most wonderful time of the year"? That thing?

Developing unrealistic expectations, or thinking about negatives that have occurred during the past year.

The holidays are when you are culturally encouraged to take stock of your life and find it desperately wanting. Conveniently, this also happens when the days are short and cold, thus casting your hopelessly flawed existence in the bleakest possible light. So, check.

Drinking too much alcohol during the holidays.

How else are you supposed to cope with the above three things?

Basically, you are screwed. If you are a human being, and you have feelings, you are going to be depressed during the holidays. The only solution is to sleep until February (which sounds like a pretty good idea no matter what), or possibly some combination of thinking only positive thoughts while expecting only misery. Paul Anthony Samuelson (May 15, 1915 -- December 13, 2009) was an American economist, and the first American to win the Nobel Memorial Prize in Economic Sciences. The Swedish Royal Academies stated, when awarding the prize, that he "has done more than any other contemporary economist to raise the level of scientific analysis in economic theory." Economic historian Randall E. Parker calls him the "Father of Modern Economics", and The New York Times considered him to be the "foremost academic economist of the 20th century."

He was author of the largest-selling economics textbook of all time: Economics: An Introductory Analysis, first published in 1948. It was the second American textbook to explain the principles of Keynesian economics and how to think about economics, and the first one to be successful, and is now in its 19th edition, having sold nearly 4 million copies in 40 languages. James Poterba, former head of MIT's Department of Economics, noted that by his book, Samuelson "leaves an immense legacy, as a researcher and a teacher, as one of the giants on whose shoulders every contemporary economist stands." In 1996, when he was awarded the National Medal of Science, considered America's top science honor, President Bill Clinton commended Samuelson for his "fundamental contributions to economic science" for over 60 years.

He entered the University of Chicago at age 16, during the depths of the Great Depression, and received his PhD in economics from Harvard. After graduating, he became an assistant professor of economics at Massachusetts Institute of Technology (MIT) when he was 25 years of age and a full professor at age 32. In 1966, he was named Institute Professor, MIT's highest faculty honor. He spent his career at MIT where he was instrumental in turning its Department of Economics into a world-renowned institution by attracting other noted economists to join the faculty, including Robert M. Solow, Paul Krugman, Franco Modigliani, Robert C. Merton and Joseph E. Stiglitz, all of whom went on to win Nobel Prizes. Samuelson was instrumental in the initial development of Indian Institute of Management Calcutta, the first Indian Institute of Management.

He served as an advisor to Presidents John F. Kennedy and Lyndon B. Johnson, and was a consultant to the United States Treasury, the Bureau of the Budget and the President's Council of Economic Advisers. Samuelson wrote a weekly column for Newsweek magazine along with Chicago School economist Milton Friedman, where they represented opposing sides: Samuelson took the Keynesian perspective, and Friedman represented the Monetarist perspective. Samuelson died on December 13, 2009, at the age of 94.

OCCUPY DC PKG

The Occupy DC rally was supposed to leave Freedom Plaza Sunday night. But on Monday morning, the protesters showed no signs of budging.

Police had not showed up to kick the activists out of their tent city on the corner of E and 14th Streets. Much of the talk on Monday morning focused on what to do in case the authorities did come to disperse the rally. That conversation would soon become unnecessary. Later that afternoon, DC Park Police told organizers that they could stay in Freedom Plaza for up to four months. The next day, DC City Council members threw their weight behind the protestors, saying they had no problem with the rallies proceeding indefinitely.

The threat of a confrontation did not scare off many participants. By 9 am, the protesters were out making coffee and planning the day's actions. The protestors planned to march to Union Station, where representatives from the Italian and Spanish Embassies were set to participate in a wreath laying ceremony to honor Christopher Columbus. They also intended to rally near the Mt. Vernon Convention Center and volunteer at a local homeless shelter, to honor World Homeless Day.

The Occupy DC protest has taken over two plazas in downtown Washington. The rally began with a dozen people in McPherson Square last week. It spread to Freedom Plaza when a separate, previously planned protest arrived on Thursday. The "Stop the Machine" rally had been in the works since the beginning of the year, and organizers secured a permit to stay in Freedom Plaza starting October 6th. Over 500 people attended that protest over the weekend, and many refused to leave Sunday night. Meanwhile, Occupy DC swelled to almost 200 people in McPherson Square, and the two protests quickly agreed to support each other.

People traveled from across the country to attend this rally, many of them leaving family and work behind. One protestor flew in from Dallas, where his stepdaughter has recently given birth to her first child. He says he is protesting on behalf of his granddaughter, so that she could grow up with a chance to succeed and lead a comfortable life. He bought a one-way ticket to Washington, and does not know when he will return to Texas.

The protest was inspired by the movement that started in New York last month. That rally was aimed specifically at eliminating the corporate greed that, in the protesters' view, is the root cause of the national depression. Most of the DC activists shared those same views, but also rallied against the war in Iraq and environmental destruction.

Never the People you Expect

This horse means the world to me. Hes my lease and I've had him about 2 months, he will go to the end of the moon for me and back. He came from Camelot 2 years ago and was really depressed ( lthe barn owner told me) then I met him. The first day I rode him I really didn't know if it'd would work out... He couldn't canter 3 strides or neck rein. Over the last couple of months we went to our first show and got reserved champion. We've worked on flying lead changes and become a stronger team in and out of the ring. What NOT to Buy Your Friend with <b>Depression</b> | <b>Depression</b> on My <b>...</b>

So, you want to get a holiday gift for your friend with depression. Let’s start with what NOT to buy.

PETS

Animal therapy is great. My dog dragged my butt out of the house when I was in the deepest throes of my last major depression. However, the time to become a pet owner is NOT when you are in the bottom of your black hole.  This is not the time to become a pack leader. Pets, especially dogs,  need affection, discipline and exercise. They need this from the moment they walk into their new home. Most of us in our healthiest state of mind aren’t up for that challenge.

Remember, puppies can read and they are discerning little rascals. Any leather product that says “Made in Italy” is as good as rawhide. I’ve never had a kitten but I hear they’re like having a little shredding machine. Ixnay on the et-pay.

SWEETS, CAFFEINE, ALCOHOL

Have you seen that ride at that fair where they put you in some kind of rubber-bank like harness and then drop you? You jump – you’re almost weightless – and you bounce up and down and up and down. This is what happens to your brain when you eat sugar. You get a spike of energy, then you crash. Spike. Crash. Spike. Crash. If you really want to give something sweet, check the labels and pick a product that has the least amount of sugar. Nuts and fruit are better than food with processed sugars.

On to caffeine. It’s a drug, pure and simple. Yes, in moderation it can be fine but if your loved one is bipolar, the last thing she needs when she is manic is a stimulant. Trust me, I know. I used to drink caffeine, especially coffee.  A few cups of Joe on top of even a mild mania and I was pawing at the ground like a racehorse in the start gate. Yes, it gave me a badly needed jump start when I was down. Still, it is a drug. Don’t believe me? Try quitting. The headache is a doozy.

Alcohol? Double-ixnay. Alcohol is a depressant, even the comfy drinks like spiked egg-nog and apple cider.  Champagne for the happy times? No. Alcohol and depression do not mix. Trust me. I’m a dual-diagnosed alcoholic. It took decades of hangovers to figure this out but and I can unequivocally say I know what I’m talking about. Alcohol is a depressant. Period.

MUSIC

Music therapy, like pet therapy is great. However, it doesn’t matter how much your friend loves Pink Floyd, Nirvana or Chopin, check the playlist. For classical music, look for anything in a minor key. During the holidays we think of Tchaikovsky’s Nutcracker and those beautiful little sugar-plum fairies. But he also wrote his Symphony No. 6 in B minor Op. 74  (aka Pathetique). Blink-182'All the Small Things makes me dance but Adam’s Song makes we want to drop a toaster in the tub. I’m just saying, check the lyrics.

BOOKS

Even if your friend loves to read, depression will shorten her attention span to that of a goldfish. As an avid reader myself, you cannot imagine my frustration at not being able to read a book. I couldn’t even make it through a magazine article. I couldn’t focus and I kept forgetting what I had read.

If you must give a book, avoid William Styron’s Darkness Visible or the biography of anyone who committed suicide. Take a good look at a self-help book before you buy it and remember, the shorter the paragraphs and sentences, the better. When I wrote my own book, Hoping for a Happy Ending: A journalist’s story of depression, bipolar and alcoholism, I kept that in mind. In my last major depression I was terrified that I would never be able to write or read a book again. So, short sentences, simple words and easy content are best. Maybe even a coffee table photo book of a place where your friend enjoyed a vacation or would love to visit.

I’m sure there are a lot of other things that don’t make good gifts for our friends with depression. So, please share them with us! Only 15 more days till Christmas…

Aberrant Neural Connections Linked to <b>Depression</b> | Psych Central <b>...</b>

Aberrant Nerve Connections Linked to Depression Major depressive disorder — also known as clinical depression — is a severe illness that affects an individual in many different ways.

The disorder is characterized by sadness, poor concentration, impaired decision-making and an inability to cope. An individual’s ability to experience pleasurable emotion can be impaired, replaced with prolonged negative thoughts and feelings.

New research, using functional magnetic resonance imaging (fMRI), gives scientists a view of how the brain changes during depression.

The findings, published in the journal Biology of Mood & Anxiety Disorders, show that a depressed brain is characterized by abnormal nerve connectivity.

Researchers from Stanford University compared the fMRI scans of women who were resting (but still awake). Half of the women were diagnosed with depression at the time of the scan and the other group consisted of women who had never had severe depression.

fMRI measures changes in blood flow, and by overlaying images of depressed and unaffected brains, a number of differences came to light.

The images showed that the depressed had decreased connectivity between several key regions of the brain responsible for emotional behavior, learning, memory and decision making.

“In addition to decreased connectivity between emotion processing regions of the brain, we found that depression was linked to an increase in connectivity between the dorsal caudate and an area of the prefrontal cortex,” said lead researcher Daniella Furman, a doctoral student in psychology.

“Deep within the brain, the caudate is thought to be involved in learning, motivation, and emotion while the prefrontal cortex at the front of the head is involved in maintaining goals and likely regulating emotional behavior. Together, these regions may act to filter out irrelevant thoughts or actions.”

Researchers believe greater connectivity may limit the depressed from updating their working memory and, as a result, have sustained negative thoughts.

Risk Factors for Recurrence of <b>Depression</b> | Brain Blogger

Depression is a common mental health disorder that negatively affects daily functioning and quality of life. Unfortunately, depression is often a recurrent disorder. Current treatment guidelines are based on clinical features of the disease, but new evidence points to sociodemographic and psychosocial characteristics that influence the severity and prognosis of depression. Specifically, a study published in the Canadian Medical Association Journal highlights smoking and a feeling of lack of control over one’s life as significant predictors of a negative long-term prognosis of depression.

The population-based survey evaluated nearly 600 people aged 16 or older who participated in a national health survey. The national survey included more than 17,000 participants, and the subjects in this evaluation were those who had reported a major depressive episode in the first year of the survey. The participants were followed for six years. More than half (64.5%) of the participants were women, and, at baseline, the average age of the group was approximately 39 years. Most of the participants were white (91.7%) and earned a middle to high income (81.5%). Nearly two-thirds had achieved greater than a high school education.

Over the follow-up period, slightly more than half (55.3%) of the survey population experienced repeated episodes of depression. The authors evaluated numerous physical, emotional, psychological, social, and demographic characteristics of the participants to examine their association with depressive episodes. Daily smoking and low mastery (feeling that life circumstances are beyond one’s control) were significant predictors of recurrent depression. A history of depression, which is already a well-established risk factor for future episodes of depression, was also significantly related to recurrent depressive episodes in this study. The association between risk factors and depressive episodes varied with the severity of depression.

Due to the nature of the study and the small sample size, the direction of effect and causal mechanisms are unclear. However, interventions that promote smoking cessation and increase mastery may provide positive mental health outcomes. Smoking cessation has obvious health benefits, and high levels of mastery are linked to regular physical activity, a healthy diet, decreased metabolic risk factors, and a decreased risk of death from cardiovascular factors. In fact, high levels of mastery at baseline were protective against recurrent episodes of depression in this population.

Smoking and mastery are simple and straightforward to assess in routine clinical practice. According to the authors of the current study, these novel prognostic factors may offer new avenues for treatment of depression and prevention of recurrent depressive episodes.

Risk Factors for Recurrence of <b>Depression</b> | Brain Blogger

Depression Anger – Why Am I Getting Angry For No Apparent Reason ...

Depression anger can come upon you out of the blue when you're suffering from clinical depression. There's often confusion over anxiety and depression and unless you've been diagnosed by a medical practitioner, you may not be aware of the nature of your particular problem. Anger in depression can be triggered by the smallest of incidents. You could be sitting in the sun with friends and the conversation turns to something non-controversial but upon which you have your own views. So you make a throwaway comment and someone comes back with a mild contradiction. This explodes in your mind into a threat and a personal insult to you as an individual, although it was never intended as such.

The innocent remark has sent you into a sudden, deep dive, changing your mood to a dark feeling of inadequacy and self-pity from which you might not emerge for several days. It's often accompanied by a feeling of hatred and loathing toward the person who made the remark. You'll probably find yourself shaking with anger and unable to make any reply. The only thing you can do at this point is to excuse yourself and walk away to allow yourself to calm down. Hopefully, you will have a close friend or a loved one who understands your problem and who knows that this depression anger can strike at any time. If possible, explain to them later how you felt and what brought it on, so that they can recognize similar trigger situations in the future.

The same feelings can be brought on by very different incidents. One example is the sound of a child crying. You could be walking around a supermarket pushing a shopping cart, when a child suddenly starts screaming in front of you. Straightaway this can bring on the feelings of depression anger and you could find yourself wanting to harm the child to keep it from crying. You need to be aware of the effect this has on you and just walk away from your cart for a time until the child has gone. It may take you a while to calm down from this but at least you have removed yourself from a potentially difficult situation.

You should understand, too, that being prone to depression anger can also leave you highly susceptible to road rage. For that reason, it may be in your best interest to keep away from crowded highways if possible. Don't avoid driving altogether, because as an activity it can be very therapeutic. Make sure you have someone in the car with you who is aware of your condition if you do plan to drive in town or on busy roads. They can advise you to pull over in the event that your depression anger is triggered.

For many years now, Jon has been helping people informally, on a personal level, to overcome depression and to get their lives back on track. After coming through a period of deep clinical depression in the early 'nineties, Jon was determined to discover for himself what lay behind this debilitating condition. For those suffering with depression, one of the biggest frustrations is the inability of friends and family to understand the depth of the problem. It's hard for loved ones to come to terms with the fact that there is no way that someone with depression can simply "snap out of it".
